Please bring the following documents:
1. State Certified Birth Certificate
2. Health Record of Required Immunization
3. Proof of Residency /3 current documents
(such as, utility bill, mortgage/lease,
insurance, or income tax return)
4. Social Security Card
5. Custody Papers (if appicable)
6. Income Verification
(Food Stamp eligibility letter, most current W-2 form,
or two most recent pay stubs)
